1. Login to
support.agtiv.com, and at AGTIV Customer
Self Service Portal landing page, click on the
Tickets link to access
ticket page:
2. Then proceed to click on Add ticket button to submit
a ticket:
*** Note ***
At Status, there are 3 links namely Open, Closed and On
Hold:
- Open – list out all open tickets
- Closed – list out all closed tickets
- On Hold – list out all on hold tickets
3. At Submit a Ticket page, users are required to field
in the mandatory fields marked with *:

- Select the classification for the issue from the
drop-down list.
- Enter the subject of the issue to be reported.
- Provide description of the issue, including
screen shots.

4. Next, continue to fill in the mandatory fields marked with * as below and click on Submit button to submit the ticket:

****Note ****
- User to provide the Symptom / Steps to reproduce
the reported issue.
- If there is additional Information, user can
utilize the Attach a file link to upload documents related to the issue,
and then proceed.
- Click on Submit button to submit a
ticket.
- Click on Discard button to cancel.

5. Once user has submitted the ticket, user will be redirected
to the ticket created as below:
